Colorado Management Memorandum of Understanding by and between Larimer County and Mandated & Nonmandated Partners (Thad Paul - Human Services) PURPOSE: House Bill 1451 legislation was intended to create a collaborative system which would allow agencies to provide more efficient and effective treatment and services to children and families in the community. The Larimer County Interagency Oversight Group (LCIOG) is the executive board charged with monitoring the provision of services, identifying gaps in services and creating new child centered, family focused, community based services. The HB 1451 legislation requires that the nine mandatory agencies participating in the project sign a yearly MOU. The MOU describes the programs funded by the project and also lists the outcome measures the programs are expected to meet. Colorado State University conducts the outcome research every year and submits s a report to the State. If the four outcomes are met, then the LCIOG receives incentive funds from the State that can be used to continue funding the programs and also create new programs as needed. Please let me know if there are other agencies or sources I should contact for any of this: · Number of kids living in out-of-home placements; · Number of children living in poverty; · Number of kids placed in congregate care; · Number of kids in kinship placements versus foster care; · Cases involving neglect; · Number of child homicides or near homicides that year; · Waiting list numbers for child care subsidies · Amount spent on in-home care; · Amount spent on out-of-home care; · Amount spent on foster care; · Length of time for dependency case resolution; · Federal and state funds used on child welfare for that year; · Percent or amount of child welfare budget that comes from federal block grants such as TANF, Social Services Block Grant and Medicaid; · Percentage or number of dependency cases that involve substance abuse; · Type of drugs recorded in dependency cases; · Number/percentage of dependency cases dismissed because findings were unsubstantiated; · Number/percentage of cases in which reunification was achieved; · Number/percentage of dependency cases re-activated within a year and within two years; · Percentage or number of dependency cases involving mental illness; · Number of families found to need services for trauma; · Number/percentage of children found to be substance exposed; · Types of drugs in cases where children were substance exposed; · Any cost analysis available regarding the state or county’s programming/court programming for that year?

Ann Marie Grobarek - Human Services Much of what you requested is not available through reports produced here at Larimer County Human Services. I have attached a spreadsheet that details what information we can provide locally and what can be obtained through other sites. Information highlighted in yellow can be obtained through reports from the Colorado Department of Human Services. You can generate public reports which can be customized by County and go back to 2006 at this site: https://rom.socwel.ku.edu/CO_Public/MyReports.aspx .

Information highlighted in gray is what I could produce locally. It is not exactly what you asked for but was the best information I could readily produce. 1. I have provided allocation information back to the State Fiscal Year 2007-08, divided out by the Child Welfare Block Allocation and Core Services/Family Preservation funds. 2. I have provided information regarding the number of children referred to trauma services back to the State Fiscal Year 2014-15. 3. The only readily available information I have regarding cost for different types of care is a slide comparing the cost of residential placement versus community based services from State Fiscal Year 2004-05 to State Fiscal Year 2015-16. 4. And I have a study showing a cost analysis on page 22 from CSU's Social Work Research Center.

Information regarding length of time for child fatalities, dependency case resolution and the number of cases that involve substance abuse or mental illness might be obtained through an open records request to the Colorado Department of Human Services at this link: https://www.colorado.gov/pacific/cdhs/child-welfare-policies-data-accountability.

Information about the total number of children in poverty is not tracked here as not all children in poverty are involved in our programs. You might be able to obtain numbers through https://demography.dola.colorado.gov/, which does break demographics down by County. This office has indicated that you should call if you are having challenges finding a report with the exact data you need.

We do not have information regarding the number of children found to be exposed to substances or the types of drugs involved as not all children exposed to substances are referred to our services. You may be able to find these numbers from the Colorado Department of Public Health at this site: https://www.ephtrequest.dphe.state.co.us/

We have reviewed your request and have determined that our office has already created part of the report you requested, therefore, that data is being provided free of charge. Attached is the current Active Tax Lien list including the additional data fields you requested.

Upon further review, our office has determined that we are not yet able to provide a list of delinquent taxes as requested. Delinquent taxes are those current year taxes which were not paid in full as of the deadlines outlined in 39-10-104.5 C.R.S. As we are currently still processing payments for the 2015 tax year, and likely will be for a couple more weeks, we are not able to provide an accurate list at this time.

Our delinquent tax statements will be generated and mailed to owners during the first week of July. If you still require this information at that time, please complete and submit a new CORA request form (http://www.larimer.org/forms/lcfitd-16.pdf).

Pursuant to C.R.S. 24-72-204(8)(a), “A designated election official shall not allow a person other than the person in interest, to inspect the election records of any person that contain the original signature, social security number, month of birth, day of the month of birth, or identification of that person, including electronic, digital, or scanned images of a person’s original signature, social security number, month of birth, day of the month of birth, or identification.” “Election records” is further defined to have the ‘same meaning as set forth in section 1-1-104(11)…and shall include a voter registration application.” C.R.S. 1-1-104(11) defines ‘election records” as, “accounting forms, certificates of registration, pollbooks, certificates of election, signature cards, all affidavits, voter applications, other voter lists and records, mail ballot return envelopes, voted ballots, unused ballots, spoiled ballots, and replacement ballots.”

Accordingly, any other documents in the Clerk and Recorder’s possession are prohibited from release.

On this solicitation (P15-36) the Evaluation Committee members unanimously selected to short-list and interview the two highest-scored vendors: Terracon Consultants, Inc. and CTL | Thompson, Inc. After interviews were complete, the Evaluation Committee unanimously chose to award to the highest-scored vendor, Terracon Consultants, Inc , for the following reasons: • Terracon demonstrated they have the staffing, experience, certifications, and procedures in place to provide the testing required by the County. • Terracon recently added GPS to their vehicles allowing them to update the County on status and location of the assigned employee(s) along with being able to send the County technicians that are closest to the current project, thus reducing time spent waiting for testing to occur. The other short-listed vendor did not indicate they had this feature. • Terracon's proprietary Construction Material Engineering and Laboratory Management System (CMELMS™) software package for reporting and data storage provides a level of customer service no other company proposed. The CMELMS™ system saves County resources/time and resolves conflicts quickly allowing county staff to focus on construction rather than paperwork.

The Evaluation Committee members individual score sheets are considered deliberative process documents and disclosure is prohibited.

This is more of a county management concern and should be responded to accordingly. I’ll remind you that Larimer County has received a percentage of its historical road and drainage budgets from federal and state governments thus binding them to the above referenced citations; Additionally, a transparency link was available on the BOCC page showing federal pass through funds from 1975-current. It is now gone. Budget has a 2005-current chart, all occurring after my last FOIA to your fiscal department. I would like a digital snapshot of the previous (1975-current) federal pass through chart that Larimer County had posted on its BOCC transparency site (This request should go to your fiscal department; accounting; etc). Please note, this information is readily available and can be e-mailed to me by your fiscal department. I have already submitted it once to DHS and what you provide me will need to match, exactly. My CORA form is attached even though this is considered an OMB A-102 jurisdiction that is technically overdue. What I'm looking for is how your County Engineer was authorized to move ahead, from two or more commissioners written admission - which I is required for public funding. I have requested, Larimer County provide a (This one should probably belong to HR and/or the Commissioners): Human resource plan for investigation and sanction concerning Mr. Peterson, if any. Mr. Peterson did not recuse or rescue; attempted to lobby the FRB during the 05.20.15 and the 09.22.15 meetings; worked for Ayers & Associates directly before the county; and, directed Mr. Koch’s contract. The 2nd part of this request involves Mr. Koch, Mr. Carlson (formerly), and Mr. Hunt (currently); all associated with Anderson & Associates and conflict with Mr. Peterson (recusal is not seen & cooperation is seen), all during 05.20.15, and some during 08.12.15 and 09.22.15. All of these members require removal from the FRB at this time. This is a FOIA as it pertains to confirmation of removal actions. I have requested from Larimer County (This one is Eric Tracy’s): A digital copy of the 09.22.15 meeting from Mr. Tracy. I have not yet received it. Larimer County Engineering has reviewed their archive records and determined that they have three sets of documents in their possession: An 11 sheet full-size mylar set of drawings for the road construction project that was conducted between old US 287 and CR 16. The plans are dated 1984. Two full-size plan sheets for a road improvement project along some portion of CR 17 (identified as being for the Hein Section of County Road 43 at the time) that are dated 1957. Plans (about 10 sheets) covering signalization at CR 14 that includes some widening and extends north for several hundred feet. The cost is $7.50 per sheet. Please advise if you wish to go forward with having copies made. It appears there are 23 sheets, which would cost $172.50. A deposit would be required before they process these copies.

2 other reconstruction projects were identified in the area:

PN 8 - CR 17 (CR 10E to CR 14) - Annexed to Berthoud PN 10 - CR 17 (CR 14 to CR 16) - Annexed to Berthoud

A few days ago, Commissioner Donnelly was quoted in an article in the Loveland Reporter Herald as basically stating that ' Larimer County has long had a policy opposing the Confederate Flag. ' At least that was the general message expressed.

I have lived in Larimer County 43 years and tend to stay informed on such matters and I do not recall any mention of this in those 43 years.

This is to be considered a request under the appropriate Colorado Statutes on public access to public records.

Please provide me the following:

1. A copy of what the County Policy is that Commissioner Donnelly referred to. 2. Explanation of what caused it to be adopted. 3. Confirmaton of the date adopted. 4. Copies of any memos, or any documentation regarding discussion, deliberation, and actions of the Commissioners in the course of adopting this claimed policy. 5. Process(es) used to develop this policy, including the involvement of the public.

Elida de los Santos did not have Mr. Eugene Scott sign her logbook but she did take a copy of the document that she notarized at that time as is permitted by notary procedures in Colorado.

There are zero entries in her logbook for August 24, 2014 There are zero entries in her logbook for September 14, 2014

Two consecutive pages of her logbook are attached for the logged documents from February 6, 2014 through March 3, 2015. There are seven logs not including Mr. Scott's document. None of the documents relate to the work of the Assessor's office and because all are related to personal matters of the individuals that appeared before Ms. de los Santos and none of these documents relate to Ms. Murray or Mr. Scott, the identifying information has been redacted.

A review of the history of the property in question shows that it has been previously been adjusted to account for condition issues. In 2001 it was adjusted from $180,500 to $162,700 for 2001/2002 cycle. In 2003 it was adjusted from $194,400 to $169,000 for the 2003/2004 cycle.

A report was generated to show the properties that were reviewed by Christine Murray and adjusted in March of 2014. Some went up and some went down

The two individuals listed on the document as witnesses are Jason Marks and Dianne Ahart. They are both employees of the Larimer County Assessor's office.

While our office does not have a department specific conflict of interest policy, we do operate under the Larimer County employee policies. Also, all of our licensed appraisers are subject to Uniform Standards of Professional Appraisal Practice as published by the Appraisal Standards Board/The Appraisal Foundation. Each appraiser is required to attend an update course on these standards once every two years.

I am writing you to formally request the accounting list that contains either monies escheated to the State or of those monies being held locally at the County level. Specifically, I am looking for a list that would contain overages from foreclosure sales and/or tax sales, unclaimed inheritance monies (estates). I would not like to receive the foreclosure list, but rather the accounting list of surplus funds aka excess proceeds, remainders, overbids, unclaimed warrants, and the same for estate funds.

As I realize that there is not usually a list specifically created to cover just these overages or estate funds, I request the following: - Any list that shows the amount of the overage, AND the case file number/descriptor. This can be done on an annual, quarterly or monthly basis(aging report), but if the monthly list changes as a

result of monies being moved to accounts, I would need the monthly lists so as not to miss monies that have been transferred to investment accounts or have been escheated to the State. I understand that this list may include monies from other sources, so a list with additional monies is fine, so long as I can tell by the file or case name what the source of the monies are. - If a list is not available to the above specifications, then a list of monies escheated to the State will suffice. Specifically, a list showing the case and/or file number or reference, and the amount escheated. As above, a list that has additional monies is fine, as long as the type of monies are ascertainable, by reference of type of case or file number.

To further clarify, I am not looking for any list that details monies that have already been disbursed from the file to individuals or companies. However, if that distinction can not be made, I would rather have the entire list and will simply check availability of the monies file by file. Escheated monies that have been sent to the State are fine.

I understand and respect the fact that this can take time to copy and/or print, and that there may be a cost associated with this. Please let me know when I can come by the court house and pick up the copies and what you believe the total cost of the copying and/or printing will be. I would also appreciate, for future reference, the name of this account or list so that I can more easily communicate my request.

In addition, if it is easier or more efficient, I am fine with receiving said information in electronic format either on disk or via email (my email address is listed below).

I appreciate any and all help you can give me in this process. I am, of course, aware of any and all State laws regarding the disposition of this money. I am also aware of the State’s Public Records Information Statutes.

Finally, I also understand that there are finder laws that can impact how these funds are handled. I am requesting this information as a public records information request for research. I will not be working as a ‘finder’ or ‘locator’. I am, at present, researching the monies being held.
