Case Management

  • Lutheran Family Services
    Care assessment and guidance to help create a plan to meet care needs and life goals. Services provided to adults 60 years and older on a sliding-fee scale.

Counseling

  • Aging Clinic of the Rockies
    • Senior Peer Counseling
      Program connects older adults age 60 and over with trained paraprofessionals, who are also seniors, to provide emotional and support services.
    • Caregiver Support Program
      Individual counseling provided to family caregivers who provide care to people 60 and over or under 60 with dementia.

Family Caregiver Support

  • Aging Clinic of the Rockies at CSU
    Individual counseling provided to family caregivers who provide care to people 60 and over or under 60 with dementia.
  • Elderhaus Adult Day Program
    Serves Larimer County adults (18 and older) with special needs and their families by providing services such as adult day services, transportation, nutrition, occupational therapy, medication administration and support groups.
  • Stepping Stones Adult Day Program
    A service of McKee Medical Center, gives participants a safe, caring place to socialize, build friendships and enjoy stimulating activities.
  • Summitstone Health Partners/Namaqua Center
    Caregiver counseling for grandparents and other relative caregivers raising grandchildren. Includes parenting classes and support groups.
  • Volunteers of America
    Caring Companions, a volunteer respite program for family caregivers.

Health Promotion

  • ColoradoVNA
    Skilled care for people 60 and over who need assistance managing their medication.
  • Ensight Skills Center
    A Matter of Balance for Low Vision - An adapted A Matter of Balance Program for the visually impaired that allows participants with vision loss to use modified materials and exercises to make learning easier. Large print materials and assistance with exercises are provided.
  • Estes Park Senior Center and Fort Collins Seniors Center
    N' Balance is an evidence-based strength training program and exercise class that focuses on re-training and strengthening your body to give you better balance. The class meets for 6 or 8 weeks, twice a week.
  • Powerful Tools for Caregivers
    A six-week educational program designed for family caregivers. Contact Lynette McGowan at 498-7758 for more information or visit the PTC national website.
  • University of Colorado Health
    Living Well with Chronic Conditions - A 6-week program, 2 ½ hours per week. Empowers people to better cope with symptoms, communicate with health care providers, eat well and exercise more.

Senior Nutrition Services

  • Estes Park Senior Center
    Provides a shelf-stable meal box (containing 9 meals) to seniors in the Estes Park Valley during the winter months.
  • Loveland/Berthoud Meals on Wheels
    Hot home-delivered meals for homebound seniors in Loveland and Berthoud. Nutrition education, counseling and screening.
  • Volunteers of America
    • Congregate meals in Fort Collins, Loveland, Wellington and Bellvue. Nutrition education, nutrition counseling and nutrition screenings.
    • Frozen home-delivered meals for homebound seniors in Fort Collins, Loveland and Bellvue. Hot delivered meals to homebound seniors in Wellington. Nutrition education, counseling and screening.

Transportation Services

  • Berthoud Area Transportation Services (BATS)
    Rides for people over 60 to medical appointments and trips that promote self-sufficiency in the Berthoud/ Loveland area.
  • Rural Alternative For Transportation (RAFT)
    Volunteer drivers will give rides to persons over 60 or adults with disabilities who cannot drive and live outside of the Town of Berthoud but within the Berthoud Fire Protection District. Rides may be scheduled to and from Berthoud, Loveland, Longmont and adjacent areas.
  • SAINT
    Rides for people over 60 to medical appointments and trips that promote self-sufficiency, in Fort Collins and Loveland.
  • Via Mobility Services
    Paratransit services five days per week for people 60 and over in Estes Park.
  • Wellington Senior Center
    Rides for people 60 and over to the congregate meals at the Senior Center and to medical appointments and self-sufficiency trips into Fort Collins.
